Notice of the No Surprises Act & Good Faith Estimates

The SLP Therapy Lab is committed to transparency and compliance with the federal No Surprises Act, which protects clients from unexpected medical bills. As part of this compliance, we provide a Good Faith Estimate of service costs to all self-pay clients.
1. Your Rights Under the No Surprises Act
You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services, including related costs like evaluations, therapy sessions, and administrative fees.
This estimate will be provided:
- In writing, prior to your scheduled service
- In a clear, understandable format
- At no cost to you
2. What the Estimate Includes
The Good Faith Estimate includes:
- Projected number of sessions
- Session fees and service codes (e.g., speech evaluation, therapy)
- Administrative or technology-related fees, if applicable
3. Dispute Resolution
If you receive a bill that is at least $400 more than your Good Faith Estimate, you may dispute the charge through a process with the U.S. Department of Health and Human Services (HHS). To initiate a dispute, you must submit a claim within 120 days of receiving the bill.

4. How to Request a Good Faith Estimate
You may request a Good Faith Estimate at any time before scheduling a service. Simply email us at info@slptherapylab.com or call us at (646) 542-3758. Estimates will be delivered within a reasonable timeframe.
